Mail or Fax Order Form

To use the following Order Form, you "must" first enter your order through our shopping cart--i.e., by clicking the Buy Button on the page of the item you wish to purchase. Then, when you click the Checkout Button, you will be given three options: (1) to pay by credit card using our Secure Online Order System, (2) to print and mail your Shopping Cart Receipt and this Order Form along with a check or money order, or (3) to fax your Shopping Cart Receipt and this Order Form along with your credit card information to us.

Please use the following Order Form ONLY if you prefer to pay by Money Order or Personal, Business or Cashier's Check; OR if you wish to pay by credit card, but choose not to use our secure order system. Simply complete the form below, then print and mail along with the Shopping Cart Receipt and your payment or credit card information. (Printing instructions are at the bottom of this form.) If you choose to fax your Order Form for faster processing, please be advised that we must receive payment or approval from your credit card company before your order is shipped. No CODs please.
